Passed peacefully on Sunday August 10, 2025 at the hospice home surrounded by her loving son, daughter-in-law and grandchildren at the age of 73 years.
Predeceased by her parents, Loi Phan & Chuc Pham; brothers, Binh Phan, My Phan, & Hieu Phan.
She is survived by her son and daughter-in-law, Sean & Julie Phan; grandchildren, Leslie, Noah, & Liam Phan; siblings, Bao Phan, Kim Phan, Thuan Phan, Hoa Phan, Thu Phan, & Toan Phan; many nieces, nephews and cousins.
Family will receive friends 4:30 pm - 8:30 pm, Thursday 14, 2025 at New Comer Cremations & Funerals, 2636 Ridgeway Avenue. Her Funeral Mass will be held 8:00 am Saturday 16, 2025 at Parish of the Holy Family, 4100 Lyell Rd, where we will proceed to White Haven Memorial Park, 210 Marsh Road, Pittsford, NY 14534 where her cremation will take place.
In lieu of flowers contributions may be made to: Mt Carmel House, 288 Frisbee Hill Road, Hilton, NY 14468.
