Passed away on, June 25, 2016 at the age of 88.
She is predeceased by her loving husband Leo.
Phyllis is survived by her son Scott, daughter Deborah, and her sister-in-law Louise.
Her grandchildren, Scott (Melissa), Theresa, Steven, and Allison, as well as many great grandchildren, nieces, and nephews.
The family wishes to extend gratitude to all the caregivers and St. Ann's staff, for the exceptional and loving care given to Phyllis over the years.

A graveside service will be held at Irondequoit Cemetery 3671 Culver Rd. Rochester, NY 14622.
